Dale Edward Phillips, age 45, of Carmichaels died November 1, 2016, in UPMC Presbyterian Hospital, Pittsburgh.

Mr. Phillips was born April 5, 1971, in Morgantown, West Virginia, the son of the late Patsy Foley Phillips and Leslie Phillips of Uniontown.

In addition to his father, he is survived by his two daughters, Heather Curfman of Waynesburg and Tiffany Phillips of Pittsburgh; his son, Kaider Phillips, at home and two sisters, Zoey Wilson and Joyce Phillips, both of Waynesburg.

Two half brothers are deceased, David A. and Jonathan D. Phillips.

Mr. Phillips was a graduate of Central Greene High School Class of 1989. He worked for Fayette County Community Action until his illness.

He will be sadly missed by his family and friends.
