Frank Kushak, 75, of Uniontown, Pa., passed away Friday, December 5, 2003 in the Uniontown Hospital.

Friends will be received in the Dearth Funeral Home, 35 South Mill Street, New Salem, Pa., on Tuesday, December 9, after 7 p.m. Tuesday, Dec. 9, 2003. Additional arrangements will be announced.
