Jacob ‘Jake’ Dale Glisan, age 20, formerly of Farmington, Pa., died suddenly on Tuesday, July 8, 2014.

He was born on Monday, July 12, 1993 in Pittsburgh, Pa., the son of Roger ‘Butch’ Glisan Sr. of Uniontown, Pa., and Kathy (Raffle) Glisan of Buffington, Pa.

Also surviving are a brother, Roger ‘Butchie’ Glisan of Fairchance, Pa; and many aunts, uncles, cousins and friends.

All services will be private for the family.
