He was born on Thursday, January 19, 1956 in Uniontown, Pa., the son of Steve Kushak and Margaret (Kermes) Kushak.
He was predeceased by an infant sister, Patricia Marie; and his parents.
He is survived by his siblings: Rosemarie Koss and her husband Tom of Upper Middletown, Pa., Jason Kushak Sr. and his wife Marisa of Upper Middletown, Pa., and their children Addison, Isabella, Jason Jr. and Kaitlyn Collins-Kushak, Timothy B. Kushak and his wife Beth Bowlen of Uniontown, Pa., and their children Hope Rose, Faith Ann, John and Becca Palmer.
