David Andrew Jack Jr., 44, of Freeport, Pa., passed away Saturday, January 13, 2018 due to complications from diabetes.

He was predeceased by his paternal grandparents, Dr. Roy C. Jack and Jo Jack of Carmichaels, Pa.

He leaves behind a son, Zander Jack; and his fianc*e, Julie Tutak of Freeport, Pa., and her children, Allyson and Braden.

Arrangements are private and under the direction of the BAKER-TERRAVECCHIA FUNERAL HOME, INC., 71 Pennsylvania Avenue, Uniontown, PA 15401 (724-438-3200).
